Hello everybody ! please introduce me, my name is Zerlina Wina Elfaretta. I am a fourth year medical student in Universitas Airlangga. During the elective block, I got an opportunity to do my posting in Hospital Canselor Tuanku Muhriz Universiti Kebangsaan Malaysia – HCTM UKM. The registration of this exchange program is through the Office of International Affairs in our faculty. This elective activity lasted for 1 month, consisting of the first two weeks I was attached at the UKM Family Medicine Center Primary Clinic in the Family Medicine department and the next 2 weeks I was attached in Hospital Chancellor Tuanku Muhriz (HCTM) in the anesthesia department.

During the first and second week I was supervized by one of the best doctor in the realm of stroke named Prof. Dr. Noor Azah Abd Aziz. She taught me many things, starting from providing an introduction to family medicine, because as we know, family medicine specialization is not a common thing in Indonesia. and also, she shared information about the health system in Malaysia which turns out to be quite different from the health system in Indonesia. In Malaysia, the first stop for anyone who is sick is not at the hospital, but at the primary clinic. she said that, as long as the disease can be treated in the clinic, the patient does not need to go to the hospital, so the hospital can focus on more advanced and serious patients.

At this primary clinic I had the opportunity to observe the doctors when treating patients in the room. I saw how doctors carry out anamnesis, dig up information about the patient’s social life, through to a physical examination and ending with giving a prescription. Apart from that, this primary clinic also holds special clinics
on certain days, such as the stop smoking clinic, diabetes clinic, and stroke clinic. In the first week I followed special clinic for stroke. I was briefed by the nurse on what to do. I assessed stroke patients using the Modified Ranking Scale (MRS), Modified Barthel Index, Twin Question with Help Quesionnaire (TQWHQ), Patient Health Questionnaire (PHQ9), and Montreal Cognitive Assessment (MOCA-BM). After going through the assessment, the patient will then be examined by the doctor.

Besides observing, I also got hands-on experience. I was attached to the treatment room, where I had the opportunity to help the medical assistant carry out treatment on patients, I had cases of changing catheters, then doing wound care, and also replacing nasogastric tubes. Not only that, in this primary clinic I also attended lectures and case presentations by master’s degree students. for presentations and lectures held every morning.

In the second to third week I rotated to the anesthesia department. While in the anesthesia department, I was supervised by one of the great doctors named Prof. Madya Dato’ Wan Rahiza Wan Mat, she is the person in charge of the ICU room. In this department I got the opportunity to see operations directly. starting from preparing the room, tools and materials, then looking at the patient’s anesthesia process, looking at the tools and drugs used for anesthesia, the operation process, to post operation care. In the first week, one of the cases I received was a patient with an aneurysm. then for the second week, I had the opportunity to observe for caesarean sections on pregnant women. Not only did I observe the operation, I also attended a visit with the anesthetist to carry out post-operative care, then I also had a visit with the doctor in the ICU. In the last week in this department I carried out observations for ECT procedures in the psychiatric ward, the patients who underwent ECT were mostly women who suffered from schizophrenia or bipolar disorder.

During the weekends, me and my friends went out to Kuala Lumpur’s tourist destinations, such as Genting Highland, Batucave Temple, Petailing Night Market, Central Market, and many more. Batucave temple is Malaysia’s iconic Hindu temple with its colorful pattern. Central Market is a market where we can go shopping for some affordable clothes, souvenirs, and other things.

This program gave me a new experience in the medical field especially in hands on experience. We can learn many things about how to interact with patients and treat the patients. This elective activity was one of my first steps to recognize and enter the real world of medicine
